Job summary

Montgomery County Coalition for the Homeless (MCCH) seeks a dedicated Case Manager to support formerly homeless individuals living in Hope Housing programs. You will maintain an active caseload, coordinate services, and apply a housing-first, trauma-informed approach to help clients achieve stable housing and life skills.

Qualifications

• Master’s degree in human services-related field required.
• 2–4 years in social/human services; diverse populations.
• Knowledge of housing first, case management principles.
• Experience with trauma-informed care and harm reduction.
• Ability to handle stressful situations and advocate for the homeless.

Responsibilities

• Maintain an active caseload of 15 clients needing intensive case management.
• Coordinate with HH case managers to ensure coverage for all HH clients.
• Implement service plans within 30 days of client entry.
• Assess clients’ needs for referrals to appropriate services.
• Conduct monthly house meetings.
• Maintain inventory and budget for supplies and equipment.
• Assist clients in accessing community resources and attending appointments.
• Monitor client progress in recovery and mental health; collaborate with providers.
