Qualifications

• Bachelor's degree in social/human or family services is required
• One year of hands-on experience in social/human services with diverse populations
• Excellent communication and interpersonal skills in English (written and spoken)
• Bilingual abilities are preferred
• Proficiency in Microsoft Office and HMIS data entry
• Ability to work cross-culturally and with individuals with limited English proficiency
• Valid driver’s license with clean driving record
• Knowledge of Housing First, Harm-Reduction, and Trauma-Informed Care

Responsibilities

• Maintain an active caseload of overflow clients who need housing-focused, individualized case management.
• Assess overflow clients’ needs and implement appropriate service plans.
• Assist overflow clients in moving to permanent housing using the rapid move-out model, aiming for 30 days when possible.
• Provide referrals to community health services, psychiatric services, vocational, educational, and substance use treatment programs, and various housing options.
• Arrange transportation for overflow clients to scheduled medical and social services appointments.
• Complete and update Acuity Scale and VI-SPDAT; maintain HMIS data and other client paperwork for reporting.
• Assess clients in crisis, resolve conflicts, and develop interventions for problematic behavior.
• Collaborate with Clinical Director, Lead Case Manager, and Operations Team for integrated service delivery.
• Maintain and review client charts to monitor progress toward permanent housing.
• Develop alternative housing options through creative problem-solving and meet clients weekly to document Case Notes in HMIS.
