Job summary

The Center for Human Development (CHD) is seeking a part-time School-Based Community Health Worker to join our Franklin County clinic in Greenfield, MA. You will provide outreach, case management, and help students and families access on-site tele behavioral health services.

You will work with a Clinician and school staff, document encounters, collect data, and complete monthly reports. The pay range is $20-$25 per hour, based on experience, with weekday hours during the school day.

Qualifications

• High School Diploma or Equivalent required.
• Strong computer skills in EMR databases.
• Knowledge of area community agencies and resources.
• Organizing resources and setting priorities.
• Creative and analytical thinking.
• Strong interpersonal and communication skills.

Responsibilities

• Provide home, school and community-based care coordination to youth and families.
• Assist students and families to access on-site tele behavioral counseling services and offsite clinicians and resources.
• Conduct comprehensive screening and assessment activities to support community-based referrals.
• Provide ongoing follow-up, motivational interviewing, and goal setting with students and families.
