Job summary

Qualifications

• Bachelor’s degree in a human service field or equivalent experience.
• Strong oral and written English communication.
• Valid driver’s license and automobile liability insurance.

Responsibilities

• Provide services in the community where the consumer may need it.
• Provide outreach, support, referral to services and connection of resources to the consumer and family.
• Develop and document treatment goals with input from client, family, and treatment team; monitor progress.
• Advocate for client rights and confidentiality; maintain ethics and privacy.
• Maintain and enter schedule and documentation in the electronic health record; meet productivity standards.
• Attend supervision and participate in program staffing; contribute to team discussions.
