Job summary

RiseMe is seeking a School Based Support Specialist to deliver intensive outpatient case management for youth in school settings, coordinating care to promote recovery, resilience, and academic success during school hours, with outreach in homes and communities as needed.

The role focuses on individualized interventions aligned with treatment plans, collaboration with schools, families, and partners, and ensuring timely documentation and client-centered services in a supportive environment.

Qualifications

• Bachelor's degree in psychology, social work, behavioral health, or related field, OR
• Two years of related behavioral health experience with a related bachelor’s degree; OR
• Four years of full-time qualifying experience in the field.
• Experience providing one-on-one or group services focused on recovery/resilience.

Responsibilities

• Deliver school-based behavioral health interventions aligned with treatment goals.
• Support coping skills, social-emotional functioning, and daily life skills.
• Coordinate care with schools, families, and providers.
• Manage caseload, ensure timely documentation and effective services.
• Provide crisis intervention and follow-up, including outreach for missed appointments.
• Conduct home visits and outreach to engage youth and families.
• Support quality assurance, data collection, and compliance with protocols.
• Participate in staff development, supervision, and evaluation.
• Adapt to evolving student needs and contribute to program goals.
• Perform other duties as assigned.
