Job summary

Centerstone is seeking a School Based Support Specialist to deliver intensive outpatient case management for youth in a school setting, coordinating care with families, schools, and providers to promote recovery and academic success.

The role focuses on individualized interventions, crisis support, home visits when needed, and data-driven service delivery aligned with treatment plans, with a strong emphasis on collaboration and professional growth within a school-based program.

Qualifications

• Experience in case management and treatment planning in mental health or substance use.
• Ability to engage youth, families, schools, and community partners.
• Knowledge of ethical, legal, and procedural standards in behavioral health.
• Proficiency with documentation and time management in busy settings.
• Ability to respond appropriately in crisis situations and maintain boundaries.

Responsibilities

• Provide school-based behavioral health interventions aligned with treatment goals.
• Support coping skills development and social-emotional functioning for students.
• Coordinate care with schools, families, and service providers.
• Manage a caseload, ensure timely documentation, and ensure effective services.
• Provide crisis intervention and outreach for missed appointments.
• Conduct home visits and outreach to engage youth and families.
• Assist with data collection and compliance with agency protocols.
• Participate in staff development, supervision, and evaluation activities.
• Adapt to evolving student needs and support program goals with flexibility.
• Other duties as assigned.

Bachelor’s degree in unrelated field with 2+ years behavioral health experience

Any four-year combination of higher education and 2+ years qualifying experience

Associate’s degree in behavioral health support

4 years full-time qualifying experience in the field
