Visiting Nurse & Hospice of Litchfield County is seeking a highly organized, dependable, compassionate full-time Scheduler to join our Home Health & Hospice team. The Scheduler coordinates patient visits for nurses, aides, therapists and other clinicians, ensuring timely, quality care and effective communication with patients and families.

The ideal candidate will manage dynamic schedules in a fast-paced environment, support admissions and follow-up visits, and maintain accurate EMR records

• Coordinate daily schedules for nurses, aides, therapists, and other clinical staff based on patient needs, staff availability, and geographic location.
• Schedule admissions, routine visits, and follow-up care.
• Communicate schedule changes with patients, families, clinicians, and leadership.
• Monitor schedules throughout the day and adjust as patient needs and staffing change.
• Assist with weekend, holiday, and on-call coverage.
• Maintain accurate scheduling information in the electronic medical record (EMR).
• Track missed, cancelled, and rescheduled visits.
• Assist with entering referrals and coordinating onboarding of new patients.
• Provide professional and compassionate customer service to patients, families, and staff.

The Scheduler plays an essential role in coordinating patient visits and supporting our clinical staff to ensure patients receive timely, consistent care.
