Job Summary:A Home HealthPhysician Orders Support Coordinator (POSC) primarily handles the efficientprocessing and management of physician orders for home health services,ensuring timely compliance and accuracy. This role involves coordinating withphysicians to obtain signed orders, tracking their status, and communicatingupdates to relevant parties, including intake and Director of Nursing/ DON. APhysician Orders Support Coordinator is also responsible to submit orders viafax or doctor’s alliance platform to PCP’s office, obtaining signed orders fora new and current patient Brockton HHA patients, upload the orders into Axxessand tracking the order's status. POSC also need to contact the physician’soffice to follow up if orders are not signed promptly. The POSC may need tocall hospitals, or PCP’s office to request last visits notes or updatedmedication list after a hospital discharge before the agency can resumeservices.
Responsibilities
New orders:fax new orders to physicians, upload orders to thepatients profile
PCP’s office and Hospital: The POSC may needto call hospitals, or PCP’s office to request last visits notes or updatedmedication list after a hospital discharge before the agency can resumeservices.
Tracking Order Status: Maintain recordsof all orders, their status (pending, signed, etc.), and ensure timelycompletion.
Communicating with Teams: Inform intaketeams and clinicians about the status of orders, including any delays orrequired actions.
Ensuring Compliance: Verify that allorders are compliant with Medicare and Medicaid also relevant regulations and BrocktonHome Health Care agency policies.
Supporting Billing: Coordinate with Clinicalmanagers and DON to ensure accurate and timely claim processing.
Maintaining Records: Keep detailedrecords of physician orders, including patient demographics, clinicalinformation, and billing details.

Excellent Communication: Strong verbal and written communication skills areessential for interacting with physicians, staff, and patients.
Organizational Skills: Ability to manage multiple tasks, prioritize work, andmaintain accurate records.
Attention to Detail: Carefully review documents for accuracy andcompleteness.
Problem-Solving: Ability to identify and resolve issues related tophysician orders and patient care.
Knowledge of Home Health: Familiarity with home health regulations, procedures,and terminology is beneficial.
Qualifications/Educational Requirements:
1.High School Education
2.Two years of college preferred.
3.Excellent written and verbal communication skills.
4.Ability to handle multiple tasks simultaneously and meetdeadlines.
5.Excellent organizational and time management skills.
