Job summary

Westside Health Authority (WHA) in Chicago is seeking a SC2 Workforce Case Manager to support participants from intake through program completion. You will provide participant-centered case management, coordinate services, and ensure accurate documentation in a cohort-based workforce development setting.

The role requires strong organization, communication, and confidentiality, with collaboration alongside the Program Manager and Employment Specialist.

Qualifications

• High School Diploma or GED required.
• Experience in workforce development, case management, intake, human services, or related field preferred.
• Experience working with individuals facing barriers to employment preferred.
• Ability to build rapport and maintain professional relationships with diverse populations.

Responsibilities

• Conduct participant intake, enrollment, orientation, and onboarding activities.
• Create, organize, and maintain participant files from intake through program completion.
• Provide ongoing case management and individualized participant support.
• Maintain regular communication with participants to promote engagement and progress.
• Monitor attendance, engagement, progress, and accountability throughout the cohort.
• Identify barriers to participation and coordinate supportive services and referrals.
• Maintain confidentiality and professional boundaries in all activities.
