Job summary

Westside Health Authority (WHA) in Chicago seeks a SC2 Workforce Case Manager to support participants from intake through program completion. You will deliver case management, coordinate services, maintain documentation, and help remove barriers to employment in a cohort-based program.

Collaborate with the Program Manager and Employment Specialist to ensure participant readiness, engagement, and successful outcomes in a grant-funded setting. Strong organization and communication are essential.

Qualifications

• High School Diploma or GED required.
• Experience in workforce development, case management, intake, human services, or a related field preferred.
• Experience working with individuals who may face barriers to employment or participation in community-based services preferred.
• Demonstrated ability to establish rapport, build trust, and maintain professional working relationships with diverse populations.

Responsibilities

• Conduct participant intake, enrollment, orientation, and onboarding activities.
• Provide ongoing case management and individualized participant support throughout program participation.
• Maintain regular communication with assigned participants to promote engagement and progress toward goals.
• Identify barriers to participation or employment and coordinate supportive services and referrals.
• Maintain professional boundaries, protect participant confidentiality, and provide culturally responsive, participant-centered service.
• Represent Westside Health Authority professionally and build positive relationships with participants and stakeholders.
