Banner Health in Torrington, WY seeks a Full Time Physical Therapist to evaluate and treat outpatient and inpatient populations in a rural setting. You will supervise assistants, coordinate with patients, caregivers, and providers, and participate in daily planning and documentation.

Day shift, Monday-Friday 8:00-5:00 with rotating weekend coverage. The role emphasizes patient-centered care and collaboration within a small, cohesive team.

Responsibilities

• Evaluates patient's functional abilities and limitations. Determines if intervention is needed. Establishes a plan of care and treatment goals. Completes comprehensive evaluation within the established time frame set by the department.
• Implements individualized treatment plan with consideration to the patient's physical, social, spiritual, cultural, educational and age specific needs.
• Documents according to professional practice guidelines set by regulatory agencies. Documents legibly and within established time frames.
• Communicates pertinent patient information to ensure the best possible follow through of care with physicians and nursing and the interdisciplinary team.
