Job summary

Lakes Region Mental Health Center, Inc. in New Hampshire seeks a Project Lead for Rural Health Transformation and CCBHC to guide strategic and operational efforts, track the work plan, and ensure certification readiness.

The role reports to the Director of Quality and Compliance and requires collaboration across departments and with external stakeholders to implement CCBHC, monitor data, and oversee grant funds and reporting.

Qualifications

• Master’s degree in behavioral health, public health, or healthcare administration.
• Equivalent experience managing federal or state-wide contracts and/or grants may be considered.
• Must have and maintain a valid NH driver’s license with current liability insurance.
• Experience with CCBHC implementation or certification preferred.
• Knowledge of SAMSHA funded programs and state regulatory requirements.
• Experience in contract funded program management.
• Demonstrated leadership experience within a health care setting.
• Knowledge of behavioral health systems, CQI, data analysis and program evaluation.
• Experience with budget oversight and financial tracking.

Responsibilities

• Lead the planning, implementation, and evaluation of all CCBHC related activities, ensuring alignment with GO-NORTH and SAMSHA certification requirements.
• Direct the CCBHC application process, including preparation, coordination, and submission to NH DHHS.
• Monitor CCBHC data collection, provide technical assistance and track related activities.
• Track action items, monitor progress and provide regular status updates to leadership and key stakeholders.
• Support organizational change management efforts associated with RHT activities.
• Ensure timely and accurate submission of all required reports.
• Maintain ongoing communication with state and federal partners, in conjunction with QI.
• Coordinate and facilitate internal and external stakeholder meetings, training, and education related to the CCBHC model.
• Collaborate with external consultants, hired or involved with RHT activities.
• Partner with senior leadership to align CCBHC implementation with organizational strategy and address operational barriers.
• Chair internal CCBHC committees, monitor project timelines, and track deliverables to ensure milestones are met.
• Assist with the development and maintenance of policies, procedures, workflows, and documentation required for certification and ongoing compliance.
• Participate in national learning collaboratives and CCBHC-focused cohorts to advance best practices and service delivery.
• Foster cross-functional collaboration to support integration of services and adherence to RHT workplan.
