Job summary

Lake Superior Community Health Center is seeking a skilled Program Manager to lead the Rural Health Transformation Program and other strategic initiatives. You will collaborate with the CEO, senior leadership, and external partners to ensure programs are delivered on time, within budget, and compliant with grant requirements.

The role serves as a central liaison across departments and helps shape future grants and program development.

Qualifications

• Bachelor's degree in a relevant field required.
• Minimum of three years of experience managing complex projects, strategic initiatives, or grants.
• Experience coordinating multidisciplinary teams and managing priorities.
• Healthcare, nonprofit, governmental, or community-based experience preferred.
• Experience with federal/state/foundation grant-funded programs preferred.
• FQHCs, HRSA programs, or value-based care knowledge preferred.

Responsibilities

• Lead the implementation of Rural Health Transformation Program (RHTP) and other initiatives.
• Coordinate development of detailed project work plans, timelines, budgets, and documentation.
• Coordinate cross-functional project teams to meet milestones and deliverables.
• Monitor project performance and recommend corrective actions.
• Facilitate project meetings, maintain action plans, and ensure accountability.
• Track budgets and monitor grant expenditures with Finance.
