RNAC (Registered Nurse Assessment Coordinator)

Horsham Center for Jewish Life of North Wales, PA, is seeking an experienced RNAC with a proven track record to join our nursing team.

Job Responsibilities

• Coordinate and develop resident assessments (MDS) according to current regulations, including RAPs and Triggers.
• Maintain and update policies related to resident assessments and care plans.
• Participate in facility surveys by government agencies.
• Evaluate residents' conditions and medical data to determine needs.
• Develop procedures with the DON to inform assessment team members of new admissions.
• Assist in scheduling assessments with care team members and family representatives.
• Perform comprehensive assessments of residents' nursing needs using required forms.
• Ensure assessment accuracy and completeness, emphasizing the importance and penalties involved.

Qualifications

• Licensed RN in Pennsylvania.
• Minimum of 2 years RNAC experience in long-term care.
• Knowledge of managed care, bundled payments, PDPM, and MDS.
• Excellent communication skills and computer literacy.
• Extensive knowledge of state and federal regulations and survey processes.
• Ability to train, oversee, and implement corrective actions.
