Job Responsibilities
- Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines, including the implementation of RAPs and Triggers.
- Maintain and periodically update written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.
- Participate in facility surveys conducted by authorized government agencies.
- Evaluate each resident's condition and pertinent medical data to determine their needs.
- Develop and implement procedures with the DON to inform all assessment team members of the arrival of newly admitted residents.
- Assist in scheduling participation by outside members of the care plan team, including the resident's representative and/or family members.
- Develop preliminary and comprehensive assessments of residents' nursing needs using the required forms.
- Ensure assessment team members understand the importance of accuracy and completeness in their assessments, and are aware of penalties for non-compliance.

Qualifications
- Licensed RN in the state of PA
- At least 2 years of RNAC experience in a long-term care setting
- Knowledge of managed care and bundled payments
- Experience with PDPM and MDS required
- Excellent oral and written communication skills; computer literacy required
- Extensive knowledge of state and federal regulations and survey processes
- Ability to provide training, oversight, and corrective actions as needed
