Job Responsibilities
  1. Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines, including the implementation of RAPs and Triggers.
  2. Maintain and periodically update written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.
  3. Participate in facility surveys conducted by authorized government agencies.
  4. Evaluate each resident's condition and pertinent medical data to determine their needs.
  5. Develop and implement procedures with the DON to inform all assessment team members of the arrival of newly admitted residents.
  6. Assist in scheduling participation by outside members of the care plan team, including the resident's representative and/or family members.
  7. Develop preliminary and comprehensive assessments of residents' nursing needs using the required forms.
  8. Ensure assessment team members understand the importance of accuracy and completeness in their assessments, and are aware of penalties for non-compliance.

  1. Licensed RN in the state of PA
  2. At least 2 years of RNAC experience in a long-term care setting
  3. Knowledge of managed care and bundled payments
  4. Experience with PDPM and MDS required
  5. Excellent oral and written communication skills; computer literacy required
  6. Extensive knowledge of state and federal regulations and survey processes
  7. Ability to provide training, oversight, and corrective actions as needed
