Qualifications

  • Licensed RN in Pennsylvania with at least 2 years of RNAC experience.
  • Extensive knowledge of state and federal regulations.

Horsham Center for Jewish Life in North Wales, PA is searching for a seasoned RNAC with a proven track record of success to join and support our nursing team!

Job Responsibilities
  • Conduct and coordinate the development and completion of resident assessments (MDS) in accordance with current rules, regulations, and guidelines, including the implementation of RAPs and Triggers.
  • Maintain and periodically update policies and procedures governing the development, use, and implementation of resident assessments (MDS) and care plans.
  • Participate in facility surveys conducted by authorized government agencies.
  • Evaluate each resident's condition and medical data to determine care needs.
  • Develop and implement procedures with the DON to inform assessment team members of new resident admissions.
  • Assist in scheduling participation of outside care plan team members, including residents' representatives and family members.
  • Develop assessments of residents' nursing needs using required forms.
  • Ensure assessment team members understand the importance of accuracy and completeness in their assessments, including awareness of penalties.
Qualifications
  • Licensed RN in Pennsylvania
  • At least 2 years of RNAC experience in a long-term care setting
  • Knowledge of managed care and bundled payments
  • Experience with PDPM and MDS
  • Excellent communication skills and computer literacy
  • Extensive knowledge of state and federal regulations and survey processes
  • Ability to train, oversee, and implement corrective actions effectively
