West Park is currently seeking a knowledgeable and compassionate Registered Nurse Assessment Coordinator (RNAC) to join our team. This is a vital clinical leadership position responsible for coordinating and overseeing the completion of accurate and timely MDS assessments, ensuring compliance with all state and federal regulations, and supporting quality outcomes for our residents.

Key Responsibilities:
Coordinate and complete the Minimum Data Set (MDS) assessments and related documentation in accordance with regulatory timelines.
Ensure accurate coding to reflect resident care and maximize appropriate reimbursement.
Collaborate with interdisciplinary team members on care plans and case reviews.
Monitor quality indicators and assist with QAPI (Quality Assurance and Performance Improvement) efforts.
Stay current with CMS guidelines, state regulations, and company policies.
Provide staff training and support on assessment practices and documentation standards.
Participate in audits, surveys, and performance reviews to maintain compliance.
Qualifications:
Active Registered Nurse (RN) license in the state of [Insert State].
Previous experience with MDS 3.0 and care planning required.
Knowledge of Medicare and Medicaid regulations, PDPM, and reimbursement guidelines.
Detail-oriented with strong organizational, clinical, and analytical skills.
Experience in long-term care or skilled nursing preferred.
