Qualifications

• Bachelor's Degree in Nursing or Social Work.
• Registered Nurse (RN) license preferred. LVN/LPN acceptable.

• Knowledge of Medicare/Medicaid programs and PCC knowledge.
• Ability to read and interpret procedures, policies and schedules.

Responsibilities

• Communicate plans of care with residents and families.
• Attend and participate in morning stand-up meetings to coordinate care.
• Answer residents' questions about care, treatment plans, and illness progression.
• Monitor and adjust resident statuses based on changing needs.
• Organize daily work and prioritize admissions and discharges.
• Complete documentation accurately and timely.
• Coordinate referrals with providers and payers.
• Oversee admissions and discharge activities and charting.
