Valley Health is seeking an experienced Unit Clinical Care Coordinator (RN) in Winchester, VA to serve as the liaison across emergency, surgical, nursing, finance, physicians, and utilization review. You will manage bed assignments, care plans, and timely progression to discharge while monitoring LOS and care levels.

The role requires strong organizational skills, crisis responsiveness, and collaboration with care teams to optimize patient flow and outcomes.

Qualifications

• BSN is required; ASN RN may qualify with a 3-year education agreement.
• MSN is preferred.Graduate of an accredited school of nursing is required.
• Licensed RN in Virginia; BLS; CCM/ACM certification within 4 years.

Responsibilities

• Coordinate patient progression and discharge planning with care teams.
• Review orders, validate level of care, and manage transfers and LOS.
• Lead SNAPHuddles to resolve progression barriers and ensure timely discharges.
• Maintain real-time updates in Care Logistics and EMR systems.
