Job summary

VCU Health seeks a Patient Flow Coordinator to optimize hospital-wide patient placement and transfers. You’ll coordinate bed assignments, triage, and referrals with clinicians, bed management, and transport teams to sustain timely, high-quality care across VCU affiliated hospitals and the community.

Requirements include a nursing diploma/ADN, RN licensure in Virginia or compact, BLS, and progression toward a BSN per policy. A Master's in Nursing is preferred for higher responsibility.

Qualifications

  • Diploma or Associate Degree in Nursing required.
  • If no Bachelor's degree, enroll in RN to BSN within two years and complete within five years.
  • Master's Degree in Nursing or related field preferred.

Responsibilities

  • Coordinate patient flow, bed placement and transfers in a timely manner.
  • Serve as a clinical and administrative resource for complex bed assignments.
  • Collaborate with physicians, Nurse Managers, charge nurses and transport teams to address flow barriers.
  • Coordinate and triage transfer referrals, including Lifecom partnerships.
  • Support psych intake referrals with psychiatric admission representatives.
  • Manage all in- and outbound calls supported by One Call Center.
  • Ensure continuity of care by facilitating transfers and transports.
