Job summary

Vail Health Hospital in Colorado Springs seeks a Triage RN to support care coordination, triage patient calls, and guide dispositions to emergency, urgent care, or clinic visits. This role relies on protocols and teamwork to provide timely, quality care.

You will document assessments in the EHR, participate in KPI-focused huddles, and contribute to quality improvement and CPC+ care coordination. A Colorado RN license or compact license is required with 3 years of clinical experience.

Qualifications

• 3 years of clinical experience preferred.
• Nursing assessment skills with clinical judgment.
• Knowledge of medical terminology, HIPAA compliance and OSHA rules.
• State of Colorado RN license or valid compact license required.

Responsibilities

• Triage calls and assess health concerns to determine disposition.
• Evaluate and prioritize patient care needs using established protocols.
• Document symptoms, assessments, and care plans in the EHR.
• Support CPC+ Care Coordination and Performance Improvement activities.
• Participate in weekly huddles to review KPI and care decisions.
• Coordinate with providers to route to emergency, urgent care or clinics.
• Ensure proper forwarding of pertinent medical information to relevant departments.
