Job summary

Mayo Clinic is seeking a Transplant Coordinator Nurse to oversee coordination of care for the transplant patient population. The role collaborates with patients, families, and a multidisciplinary team to plan, implement, and evaluate care across the continuum.

A strong emphasis on education, leadership, and adherence to ANA/UNOS guidelines is required. Qualified candidates hold a Baccalaureate ACEN/CCNE-accredited nursing degree, with 1 year RN experience (3 years preferred).

Qualifications

• Graduate of ACEN- or CCNE-accredited nursing program.
• Minimum 1 year of clinical RN experience required; 3 years preferred.

Responsibilities

• Coordinate all aspects of care for transplant patients in collaboration with patient, caregiver, and multidisciplinary team.
• Assess data and plan according to age-specific needs and care continuum.
• Provide patient and family education on a consistent basis.
• Provide leadership through committee work and quality improvement activities.
