Job summary

Mayo Clinic in Minnesota is seeking a Transplant Coordinator Nurse to coordinate all aspects of care for the transplant patient population. You will assess and plan with patients, families, and the multidisciplinary team to meet needs across the care continuum.

You will provide patient and family education, lead quality improvement activities, and adhere to ANA/UNOS/FACT guidelines. Eligible for TN sponsorship; state licensure required and completion of VisaScreen or equivalent certification.

Qualifications

• Graduate of a BSN program accredited by ACEN or CCNE.
• Minimum 1 year of clinical RN experience; 3 years preferred.

Responsibilities

• Coordinate all aspects of care for transplant patients with patient, caregiver and multi-disciplinary team.
• Assess patient status and plan care across the continuum.
• Provide patient and family education on a consistent basis.
• Provide leadership through quality improvement, committee work and research.
