Job summary

Endeavor Health in Arlington Heights, IL, is seeking a RN Transitional Care Navigator for full-time, 40 hours/week. The role includes case management, care coordination, and utilization management across care settings with on-site 2 days and remote 3 days per rotation.

Responsibilities include guiding high-risk patients through the health system, creating individualized care plans, collaborating with the care team, and monitoring utilization and cost of care while supporting smooth transitions

Qualifications

• Bachelor's degree in healthcare administration or related field.
• Experience in utilization review, discharge planning, case management or disease management recommended.
• Home/hospital nursing experience beneficial and preferred.

Responsibilities

• Guides high-risk patients and families through the health system from diagnosis to follow-up.
• Establishes individualized care plans using evidence-based guidelines.
• Collaborates with the healthcare team to ensure timely decisions and discharge planning.
• Coordinates daily across departments to ensure safe transitions and cost management.
• Educates patients and families on resources, medications, and follow-up care.
• Facilitates appointments within established protocols and monitors utilization.
• Applies clinical guidelines to monitor admissions and LOS, improving efficiency.
