Job summary

Endeavor Health is seeking a Transitional Care Navigator in Illinois to manage case coordination across care settings. The role emphasizes improving costs, reducing readmissions, and optimizing transitions for high-risk populations.

You will partner with the care team to implement evidence-based plans and ensure seamless patient journeys. The position requires RN licensure in Illinois, and offers a mix of on-site and remote work with a schedule of Monday–Friday, 8:00am–4:30pm, including some

Qualifications

• Bachelor's Degree in Healthcare Administration required.
• Nursing degree preferred.
• 3+ years in utilization review, discharge planning, case management or disease management preferred.
• Nursing experience in home/ambulatory services beneficial.
• 2+ years of clinical nursing experience preferred.

Responsibilities

• Guide high-risk patients through the health system from diagnosis to follow-up.
• Create and document individualized care plans.
• Coordinate discharge planning with the care team.
• Conduct daily coordination across departments to ensure patient safety and cost-effective care.
• Provide education to patients and families on medication management and resources.
• Arrange appointments and support services as per protocols.
• Perform Utilization Management and monitor LOS and resource use.
• Travel to visit patients at home as needed.
