Job summary

Endeavor Health is seeking a Transitional Care Navigator in Arlington Heights to oversee case management and care coordination across multiple care settings. The RN role emphasizes guiding patients through diagnosis, treatment choices, and resource access, with a focus on reducing readmissions and optimizing care transitions.

The position is full-time with 40 hours weekly, featuring a hybrid work arrangement: two days onsite and three days remote, plus weekend/holiday rotations.

Qualifications

• Bachelors Degree Healthcare Administration required.
• Bachelors Degree Nursing preferred.
• 3 years minimum utilization review, discharge planning, case management or disease management preferred.
• RN experience in home or ambulatory settings beneficial.
• 2+ years clinical nursing experience preferred.
• Clinical certifications beneficial.

Responsibilities

• Guides high-risk patients and families through diagnosis, testing, treatment, and follow-up, helping navigate continuum of care.
• Documents and maintains an individualized care plan aligned with patient goals and evidence-based guidelines.
• Partners with healthcare team to ensure timely clinical decision-making and discharge planning.
• Coordinates daily across departments and clinics to ensure patient safety and efficient transitions of care.
• Educates patients and families on medications, resources, and decision-making; provides emotional support.
