Job summary

Endeavor Health is seeking a Transitional Care Navigator (RN) to manage case coordination across care settings in Arlington Heights. The role emphasizes guiding patients from diagnosis through follow-up, coordinating with the care team, and ensuring safe transitions.

You will engage with social work for counseling and educate patients on resources and plans of care. Strong RN background and utilization management experience are required; Illinois RN licensure and AHA BLS preferred.

Qualifications

• Bachelor's degree in healthcare administration required.
• Nursing experience or degree preferred, with 2+ years in clinical nursing preferred.
• 3 years minimum in utilization review, discharge planning, case management or disease management preferred.
• Experience with high-risk patients and home/ambulatory services beneficial.
• IDFPR RN licensure in Illinois required.
• Proficient in Microsoft Office and community resources.

Responsibilities

• Guide high-risk patients through the health system and care continuum.
• Coordinate care transitions and discharge planning with a multi-disciplinary team.
• Conduct utilization management and review to improve cost and outcomes.
• Educate patients and families on medications, resources, and plans of care.
• Arrange appointments and support services per established protocols.
• Monitor length of stay and resource use; participate in data collection for utilization.
