Job summary

Endeavor Health in Arlington Heights, IL is seeking a Transitional Care Navigator (RN, Population Health) to lead case management and coordination across care settings. The role focuses on guiding high-risk patients, optimizing transitions, and reducing readmissions, with a 40-hour workweek and a mix of on-site and remote coverage.

You will work with the care team to develop individualized care plans, monitor utilization, and ensure cost-effective outcomes.

Qualifications

• Bachelors Degree Healthcare Administration required.
• Bachelors Degree Nursing preferred.
• 3+ years in utilization review, discharge planning, case management or disease management preferred.
• RN licensed in Illinois IDFPR.
• Strong communication and data entry skills.
• Experience with InterQual or MCG criteria preferred.

Responsibilities

• Guide high-risk patients through the health system from diagnosis to follow-up.
• Develop individualized care plans using evidence-based guidelines.
• Coordinate discharge planning with the care team.
• Facilitate transitions and ensure patient safety and resource use.
• Collaborate with social work on education and counseling.
• Arrange appointments and support services per protocol.
• Monitor utilization and total cost of care; analyze data.
