The RN Transitional Care Navigator coordinates care across settings, facilitates transitions, and collaborates with the healthcare team to improve outcomes and reduce costs.

This full-time position in Arlington Heights combines on-site and remote work, with travel to patient homes as needed and a focus on

Qualifications

• Bachelor's degree in Healthcare Administration required.
• RN licensed in Illinois preferred.
• 3+ years in utilization review, discharge planning, or case management.
• Nursing experience in home or ambulatory settings beneficial.
• Strong communication and collaboration with stakeholders.
• Knowledge of community resources.
• Experience with EMR.
• Ability to travel to patient homes.

Responsibilities

• Guide high-risk patients through diagnosis to follow-up care.
• Develop individualized care plans.
• Coordinate discharge planning with care team.
• Monitor utilization and total cost of care.
• Educate patients and families on resources and treatment options.
• Arrange appointments and support services.
• Perform Utilization Management using Milliman Guidelines.
• Travel to patient homes as needed.
• Participate in data collection to improve care and costs.
