Job summary

AdventHealth in Winter Park, FL is seeking a Registered Nurse to identify patients at risk for readmission and collaborate with the care team to ensure safe transitions of care. You will educate patients and families on disease self-management during hospitalization and after discharge, and coordinate post-acute resources to support recovery.

Requirements include RN licensure and 1+ year in nursing; Bachelor’s or Master’s in Nursing preferred.

Qualifications

• RN licensure required.
• 2+ years in care management, chronic disease management, or care coordination in healthcare.
• Experience in outpatient or home health and critical care preferred.

Responsibilities

• Identify patients at risk for readmission and coordinate safe transitions of care.
• Educate patients and families on disease self-management during hospital stay and after discharge.
• Assess medication adherence and provide education to improve compliance.
• Coordinate post-acute resources for patients needing support post-discharge.
• Collaborate with the care team and present readmission prevention data.
