Job summary

Avera Palm Place is seeking an RN coordinator to manage patient transfers across our regional facilities. You will serve as the single point of contact, coordinating with nursing teams and physicians to ensure smooth transitions and continuity of care.

You will monitor transfers, identify bottlenecks, and communicate with care teams to optimize patient placement while supporting a collaborative, mission-driven environment.

Qualifications

• Active RN license in state of practice.
• Strong communication and teamwork abilities.
• Ability to work in rotating shifts and handle transfers efficiently.

Responsibilities

• Oversees patient flow from outlying facilities to ensure seamless admission into the facility.
• Prioritizes and places patients in appropriate regional sites using policies and guidelines.
• Works with bed placement coordinators to ensure timely transfers.
• Evaluates transfer processes to improve outcomes.
• Collaborates with physicians, nurses, and other healthcare providers to assess patient placement.
