Job summary

Avera Health is seeking an RN Coordinator to manage patient transfers across the system, acting as the central point for placement decisions. You will work within a multidisciplinary team to coordinate timely admissions and ensure continuity of care, collaborating with physicians, nurses, and facilities to move patients efficiently.

The role requires an active RN license, good communication, and the ability to navigate rotating 12-hour shifts including weekends and holidays.

Qualifications

• Active RN license in the state of practice.
• Preferred 1-3 years nursing experience.
• Strong communication and teamwork skills.

Responsibilities

• Oversees patient flow from outlying facilities to ensure timely entry.
• Prioritizes and places patients at appropriate regional sites.
• Acts as liaison with bed placement to ensure timely transfers.
• Evaluates transfer center processes toward desired outcomes.
• Collaborates with physicians and nurses to assess patient placement.
• Uses healthcare technology to gather data and support care coordination.
