Job summary

Avera Palm Place in Sioux Falls, SD is seeking an RN coordinator to serve as the single point of contact for transferring patients within our health system. You will collaborate with physicians, nurses, and bed placement teams to identify the most appropriate facility and ensure timely movement of patients.

The role requires an active RN license, strong communication, and problem-solving abilities, with rotating 12-hour shifts.

Qualifications

• Active Registered Nurse (RN) license in the state of practice.
• Strong communication skills and ability to convey information clearly.
• Ability to collaborate with physicians, nurses, bed placement, and other care team members.

Responsibilities

• Oversees the flow of patients from outlying facilities to ensure efficient entry into the facility.
• Prioritizes and places patients in appropriate regional sites.
• Acts as liaison with bed placement coordinator to ensure timely transfers.
• Evaluates transfer processes to improve outcomes.
• Collaborates with physicians and nurses to assess patient placement.
• Utilizes healthcare technologies and information systems as needed.
