Job summary

You’ll lead a care team, perform assessments, coordinate plans, and ensure continuity of care from admission through discharge.

Qualifications

• Must hold a current, valid state RN license.
• Current CPR certification required.

Responsibilities

• Lead direct care providers to ensure medications and treatments, documentation, family teaching, care planning and patient care conferencing per campus policies.
• Utilize the nursing process to deliver care and ensure continuity from admission through discharge.
• Visit residents daily to assess health status and notify nursing leadership if changes occur.
• Identify emergencies and initiate lifesaving measures when physician is unavailable.
• Perform administrative duties such as completing medical forms, reports, evaluations, and charting as needed.
