Qualifications

Responsibilities

• Supervise clinical care planning and delivery to ensure coordinated resident care and updated care plans.
• Assess residents on admission and during changes in condition.
• Conduct regular rounds to evaluate resident status and identify needs.
• Provide or assist with direct resident care including ADLs, meals, medication and treatments, and activities as appropriate.
