Qualifications

Responsibilities

• Assist in implementing and revising care plans, monitor health conditions, provide direct care, and respond to emergencies or changes in status.
• Communicate changes in resident conditions to families/providers and work collaboratively with the interdisciplinary team.
• Review and investigate incidents, transcribe orders, audit medical charges, and maintain record compliance for accuracy.

• Pay: $95,000 annually (negotiable with experience)

Responsibilities

• Assist in implementing and revising care plans, monitor health conditions, provide direct care, and respond to emergencies or changes in status.
• Observe care assignments, manage coverage needs, evaluate staff performance, educate and lead staff.
• Communicate changes in resident conditions to families/providers and work collaboratively with the interdisciplinary team.
• Review and investigate incidents, transcribe orders, audit medical charges, and maintain record compliance for accuracy.
• Explain health status, changes, and expectations for care; guide and direct care skills with intervention as needed.
