Job summary

Avamere is seeking a Registered Nurse (RN) to provide comprehensive patient care at 625 Stevens St., Medford, OR. You will assess residents, administer medications, monitor health, and coordinate care with the team.

The role includes supervising LPNs and CNAs, maintaining documentation, and participating in interdisciplinary rounds. CPR/BLS is required; a BSN is preferred, with LTC or geriatric experience a plus.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, unencumbered, active license to practice as an RN in Oregon. BSN preferred.
• Must maintain an active CPR/BLS during employment.
• Prefer experience in long term care, geriatric rehab, sub-acute or critical care.

Responsibilities

• Assess residents and develop individualized care plans with the interdisciplinary team.
• Administer medications and monitor responses with accurate documentation.
• Supervise LPNs and CNAs and provide orientation and training.
• Maintain accurate resident care documentation and communicate with families.
• Participate in interdisciplinary meetings and quality improvement initiatives.
