Job summary

Avamere Skilled Advisors, LLC is seeking a Registered Nurse (RN) for full-time day and noc shifts at Avamere Three Fountains in Medford, OR. You will provide comprehensive resident care, including assessments, medication administration, and coordination with the care team.

The role requires an active Oregon RN license, a BSN preferred, and CPR/BLS. Experience in long-term care or geriatric settings is a plus. We offer a comprehensive benefits package and equal opportunity employment.

Qualifications

• Must maintain and have an active CPR/BLS during employment.
• Experience in electronic medical records documentation.

Responsibilities

• Assess residents and develop individualized care plans with the interdisciplinary team.
• Administer medications and monitor patient responses.
• Supervise and train licensed practical nurses and CNAs; provide orientation.
• Maintain accurate documentation and communicate with families and healthcare professionals.
• Participate in quality improvement initiatives and stay current with best practices.
