Hospital for Special Care is seeking an RN Shift Clinical Coordinator for the Satellite Units at Mount Sinai. You will coordinate patient care activities, lead the nursing team, and collaborate with other disciplines to ensure high-quality outcomes on your shift.

The role emphasizes leadership, clinical expertise, and education of staff within a 40-hour week on the third shift, with emphasis on patient safety and care continuity.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure as a registered nurse in the State of Connecticut.
• 3–5 years recent clinical experience in related setting.
• Demonstrated competence in the clinical setting.
• Ability to collaborate with an interdisciplinary team, critical thinking, and leadership skills.
• BSN
• Valid/Current CPR

Responsibilities

• Performs comprehensive patient assessments/reassessments.
• Directs and supervises the patient care activities of the patient care team.
• Coordinating patient centered patient care activities with the treatment team.
• Facilitates continuity from shift to shift for each patient’s plan of care.
• Mentors staff related to assessments, plan of care, interventions, and communication.
