Job summary

The nurse will utilize Relationship Centered Care within the Modified Primary Nursing model, participate in admissions and discharge activities, and collaborate with the interdisciplinary team to ensure high-quality outcomes.

Qualifications

• Graduation from an accredited nursing program and NY RN license.
• BSN preferred; ADN acceptable with CNO approval.
• Current BCLS, NRP, ACLS, and PALS certifications.

Responsibilities

• Delivers patient care using Relationship Centered Care and nursing process.
• Assesses and evaluates patient care needs with critical thinking.
• Collaborates with the interdisciplinary team in developing, implementing, and evaluating the plan of care.
• Prioritizes patient care activities including teaching, rounding, and post-discharge planning.
• Educates patients and caregivers about plan of care and transition of care.
