Job summary

One Senior Care is seeking a Registered Nurse to advocate for participants and navigate their healthcare journeys. You will deliver direct and indirect nursing care, manage complex clinical situations, and ensure safe, effective care delivery.

This role is ideal for a results-driven, organized professional who thrives in a fast-paced healthcare environment and collaborates with a care team to support participant well-being. Home visits and on-call rotation are required.

Qualifications

• Graduate of accredited nursing program with active RN license.
• Experience in complex nursing care and interdisciplinary collaboration preferred.
• Experience with frail/elderly population is a plus; training provided on request.

Responsibilities

• Provide direct and indirect nursing care to participants per Care Plan.
• Collect and analyze patient data, set priorities, and identify follow-ups.
• Develop, implement, and evaluate plans of care with interdisciplinary teams.
• Educate participants and families; provide discharge instructions.
• Respond to emergencies and provide advanced care as needed.
• Document care and outcomes in the EMR accurately.
• Communicate participant changes to the care team.
• Participate in Quality Improvement initiatives and process enhancements.
• Travel to participant homes or other sites as needed.
