Job summary

One Senior Care seeks a dedicated Registered Nurse to provide direct and indirect nursing care for participants, both in-home and in clinical settings. You will be part of a collaborative care team, managing complex clinical situations while ensuring safety and quality of care.

The role emphasizes urgency, precision, and communication, with 20% travel to participant homes. Training and mentorship support growth and success in a fast-paced healthcare environment.

Qualifications

• Graduate of accredited nursing school and current RN license.
• Experience in complex nursing care and interdisciplinary collaboration preferred.
• Experience with frail or elderly population is a plus, with training provided.

Responsibilities

• Provide direct and indirect nursing care following the care plan.
• Collect and analyze patient data, set priorities and identify follow-up needs.
• Develop, implement, and evaluate plans of care with interdisciplinary teams.
• Educate and discharge instructions to participants and families.
• Respond to emergencies and provide advanced care as needed.
• Document care and outcomes in the EMR accurately.
• Communicate changes to the care team clearly.
• Participate in quality improvement initiatives and clinical process enhancements.
• Travel to participant homes or other sites as needed to provide care.
