One Senior Care is seeking a Registered Nurse to provide direct and indirect nursing care to participants, manage complex clinical situations, and ensure safe, effective care. The role emphasizes independence, collaboration with interdisciplinary teams, and a focus on high-quality outcomes.

The position involves about 20% home visits, first shift, full-time hours, and on-call rotation for evenings, weekends, and holidays.

Qualifications

• Must be a graduate of an accredited nursing school.
• Current RN license in practicing state.

Responsabilités

• Provide direct and indirect nursing care to participants following the Care Plan.
• Collect and analyze patient data, set priorities, and identify problems needing follow-up.
• Develop, implement, and evaluate plans of care with interdisciplinary teams.
• Provide education, guidance, and discharge instructions to participants and families.
• Respond to emergencies and implement advanced care as needed.
• Document care, observations, and participant outcomes accurately in the EMR.
• Communicate participant changes and updates to the care team.
• Participate in Quality Improvement and clinical process enhancements.
• Travel to participant homes or other sites as needed.
