Job summary

One Senior Care is seeking a Registered Nurse to provide direct and indirect nursing care to participants, including home visits. You will manage complex clinical situations, collaborate with a care team, and ensure safe, effective care delivery in a fast-paced environment.

The role emphasizes urgency, precision, and problem-solving, with a focus on participant well-being and professional development through training and mentorship.

Qualifications

• Must be a graduate of an accredited nursing school and have a current RN license within practicing state.
• Experience in complex nursing care and interdisciplinary collaboration preferred.
• Experience working with the frail or elderly population is a plus — but not required. We’ll provide hands-on training and mentoring to help you grow!

Responsibilities

• Provide direct and indirect nursing care to participants following the Care Plan.
• Collect and analyze patient data, set priorities, and identify problems needing follow-up.
• Develop, implement, and evaluate plans of care, collaborating with interdisciplinary teams.
• Provide education, guidance, and discharge instructions to participants and families.
• Respond independently to emergencies and implement advanced care as needed.
• Document care, observations, and participant outcomes accurately in the electronic medical record.
• Communicate participant changes and updates clearly to the care team.
• Participate in Quality Improvement initiatives and clinical process enhancements.
• Travel to participant homes or other sites as needed to provide care.
