Job summary

One Senior Care seeks a dedicated Registered Nurse to deliver direct and indirect nursing care to participants, following individualized care plans. You will collect data, prioritize needs, and work with interdisciplinary teams to implement and evaluate plans of care.

Travel to participant homes is required, with emphasis on timely, accurate documentation and clear communication. Training and mentorship are provided to support your growth and success in a fast-paced home-health setting.

Qualifications

• Must be a graduate of an accredited nursing school and have a current RN license within practicing state.
• Experience in complex nursing care and interdisciplinary collaboration preferred.
• Experience working with the frail or elderly population is a plus — but not required. We’ll provide hands-on training and mentoring to help you grow!

Responsibilities

• Provide direct and indirect nursing care to participants following the Care Plan.
• Collect and analyze patient data, set priorities, and identify problems needing follow-up.
• Develop, implement, and evaluate plans of care, collaborating with interdisciplinary teams.
• Provide education, guidance, and discharge instructions to participants and families.
• Travel to participant homes or other sites as needed to provide care.
• Document care, observations, and participant outcomes accurately in the EMR.
