Job summary

One Senior Care is hiring a Registered Nurse to provide direct and indirect nursing care to participants, including home visits. You will assess, plan, implement, and evaluate care with a collaborative team to support safe, effective care.

The role requires RN licensure in the practicing state and experience with complex nursing care. Training and mentoring are provided, with a strong emphasis on timely decision-making and high-quality patient education.

Qualifications

• Must be a graduate of an accredited nursing school and have a current RN license in practicing state.
• Experience in complex nursing care and interdisciplinary collaboration preferred.
• Experience with frail or elderly population is a plus, but not required.

Responsibilities

• Provide direct and indirect nursing care to participants following the Care Plan.
• Collect and analyze patient data, set priorities, and identify problems needing follow-up.
• Develop, implement, and evaluate plans of care in collaboration with interdisciplinary teams.
• Provide education, guidance, and discharge instructions to participants and families.
• Respond independently to emergencies and implement advanced care as needed.
• Document care, observations, and participant outcomes accurately in the electronic medical record.
• Communicate participant changes and updates clearly to the care team.
• Participate in Quality Improvement initiatives and clinical process enhancements.
• Travel to participant homes or other sites as needed to provide care.
