Curana Health is seeking a Care Ally, Case Manager to join our interdisciplinary care team. You will assess member health needs, develop and coordinate a plan of care, and serve as a liaison among members, their RP/POAs, and care providers.

You will educate members and families and help navigate benefits and community resources. You will work in a hybrid role, traveling to facilities 2–3 days per week with remote work the rest of the time, collaborating with PCPs, APPs, and support teams to

Qualifications

• One year of clinical practice experience in case management, home health, critical care, medical/surgical, discharge planning, or concurrent review.
• Proficiency with Microsoft Office (Word, Excel, Outlook) and ability to navigate multiple systems.
• Case management certification preferred; long-term care or nursing facility experience preferred.
• RN or LPN license in the state where job duties are performed; or BS in social work; MSW licensure preferred but not required.

Responsabilités

• Conduct comprehensive health assessments and develop a plan of care with the interdisciplinary care team.
• Educate members and families as health coaches on disease status, plan benefits, resources.
• Coordinate care with PCPs and care team to facilitate appropriate treatment.
• Support member and RP/POA with scheduling and arranging appointments and services.
• Monitor care plans and update the team on member progress and plan changes.
