Job summary

SourcePro Search, LLC seeks a proactive Risk Manager / Assistant Director of Nursing (ADON) to lead clinical quality and risk mitigation in a premier long-term care facility.

The role requires direct experience investigating, reporting, and submitting accident and incident reportables to the Department of Health (DOH); you will partner with the DON to oversee daily nursing operations and ensure compliance with DOH regulations while mentoring staff.

Qualifications

• Active, unrestricted RN license in the state of practice.
• Hands-on experience submitting accident and incident reportables to the DOH.
• 3+ years RN experience in a long-term care facility.
• Experience as ADON / Risk Manager / QIC / Clinical Supervisor in long-term care.
• Deep understanding of DOH reporting mandates and state surveys.

Responsibilities

• Direct the investigation, documentation, and mandatory reporting of all facility accidents, incidents, and reportable events to the Department of Health (DOH).
• Partner with the Director of Nursing (DON) to oversee daily nursing operations and maintain high clinical quality standards across the facility.
• Conduct thorough root-cause analyses on clinical incidents and implement immediate corrective action plans.
• Ensure continuous compliance with federal, state, and local DOH regulations and lead state survey readiness.
• Educate and train nursing staff on safety protocols, risk reduction, and proper incident documentation.
