Talent With Heart is seeking a proactive Risk Manager / Assistant Director of Nursing (ADON) to lead clinical quality and risk mitigation in a premier long-term care facility. The role requires hands-on experience investigating and reporting to the Department of Health (DOH).

Responsibilities include directing investigations, partnering with the DON, and ensuring compliance with DOH regulations. The successful candidate will mentor staff and drive safety and quality improvements across all

Qualifications

• Active, unrestricted RN license in the state of practice.
• Must have documented hands-on experience submitting accident and incident reportables to the DOH.
• 3+ years of RN experience in a long-term care, skilled nursing, or sub-acute facility.
• Prior experience as an ADON, Risk Manager, Quality Improvement Coordinator, or Clinical Supervisor in long-term care.
• Deep understanding of DOH reporting mandates, state surveys, and risk containment.

Responsabilités

• Direct the investigation, documentation, and mandatory reporting of all facility accidents, incidents, and reportable events to the DOH.
• Partner with the Director of Nursing to oversee daily nursing operations and maintain high clinical quality standards across the facility.
• Conduct thorough root-cause analyses on clinical incidents and implement immediate corrective action plans.
• Ensure continuous compliance with federal, state, and local DOH regulations and lead state survey readiness.
• Educate and train nursing staff on safety protocols, risk reduction, and proper incident documentation.
