Rogue Valley Manor is seeking an experienced Resident Care Manager/MDS Coordinator - RN to lead a skilled nursing team and coordinate comprehensive resident care. You will oversee admissions, care planning, and interdisciplinary collaboration to ensure high-quality, person-centered services.

The role emphasizes regulatory readiness, MDS coordination, and ongoing staff development, with a Monday–Friday schedule and salary DOE.

Qualifications

• 1+ year of professional nursing experience in a skilled nursing setting.
• RN license from an accredited program is required.
• Supervisory experience in a similar setting including performance management.
• Ability to work independently and make sound judgments in resident care.
• Knowledge of state, federal and local long-term care regulations.
• Current CPR certification.
• Knowledge of EMR systems and general computer proficiency.

Responsabilités

• Lead and inspire a high-performing nursing team and foster a mission-driven culture.
• Advocate for resident rights and deliver resident-centered care promoting dignity and independence.
• Coordinate care across physicians, nurses, therapists, managed care partners and the interdisciplinary team.
• Oversee admissions, assessments, care planning, care conferences and discharge planning.
• Monitor resident conditions, physician orders, incidents and clinical documentation for quality care and compliance.
• Drive quality improvement initiatives and support regulatory readiness.
• Partner with families and healthcare representatives for collaborative care planning.
• Support MDS coordination, PDPM and OBRA compliance, utilization review and reimbursement processes.
• Mentor and develop nursing staff through coaching and accountability.
• Provide leadership support including on-call coverage.
• Perform other duties as assigned
